Here for those who need us. Healthy Horizons is a 12-week program designed to promote physical health and well-being and encourage community connection for those in the mental health recovery process.
Pathway participants will graduate with supportive relationships, as well as the education and tools necessary to continue their health and wellness journey.
The Program
This free program includes:
✔️ Participation in 12-week Healthy Horizons Program working on goal setting and tracking while learning about exercise, nutrition, meditation and mindfulness, and a variety of movement types.
✔️ Membership for participant and support system (family, friend, sponsor, etc.) during the 12-week duration of the Healthy Horizons Program.
✔️ 3-month Adult memberships for program participant after successful completion of the 12-week program
Who qualifies:
✔️ Those who have been diagnosed with a mental health disorder and are in the recovery process*
* Y-provided referral form must be filled out by a health care provider and submitted prior to participation. Participants must be in the “maintenance stage” of recovery and 60 days from last use. For all other mental health diagnoses, a letter from a health care provider is required to confirm diagnosis and status of “in the recovery process."

Our Healthy Horizons Program has impacted 132+ individuals and their families since program inception in 2019.

Notes on the Pathway Experience
In our efforts to provide a platform of togetherness and safety, and by the graciousness of our donors, your pathway experience will consist of five, important modules:
- Access to our virtual wellness platform, Y Wellness 24/7
- Weekly connections and conversations via ZOOM with your pathway family.
- Pre/Post survey and InBody scan
- Group Togetherness sessions are provided to participants of all pathway programs, offering valuable information and more.
- Note: In valuing the graciousness of our donors, successful enrollment in a pathway program relies on consistent attendance and participation. Pathway participants with 3 or more class absences without communication to the instructor will be disenrolled so we may provide an opportunity to another. Kindly consider this prior to joining us.
